Name Changes
Name changes are processed by the Department. The licensee must complete a 2C form and mail it to the Department with the $25.00 filing fee.

Discount Medical Plan Organizations
All Discount Medical Plan Organizations are required to report the most recent audited and most current un-audited financial statement to the Department by March 31st of each year. Please fax this information to (302) 736-7906 or email to licensing@delaware.gov attention Producer Licensing. Include the full name, license number and FEIN of the DMPO on the submission.

Licensing Procedures. The Department will only accept electronic license and renewal applications (please see Agent Bulletin No. 15). Applicants may apply online through the NIPR (National Insurance Producer Registry). The NIPR Gateway is a communication network that electronically links state insurance departments with the entities they regulate. The website address to submit an application is: www.nipr.com.
